Referring a Patient

Please direct any patients to complete the screening form online or contact one of our intake coordinators at 212-378-4545 for Outpatient Services or 212-293-3000 for Inpatient Services.

At ACI, our admissions team works diligently to provide quality services that meet the needs of newly admitted patients. All patients are screened and assessed to determine their appropriate course of treatment. The admissions team assess for the following factors when a patient is considered for treatment:

  • Co-Occurring Disorders
  • Recent Hospitalizations
  • Preexisting Medical Conditions
  • Suicide History
  • Family History
  • Substance Abuse History

ACI ensures that our patients receive comprehensive treatment services and are provided with appropriate levels of treatment that suit their needs.

"I used to be a successful teacher. I was proud of myself and my achievements. I started using drugs and hit rock bottom. Then a friend took me to ACI. I went to detox and rehab. I am currently enrolled in the outpatient program and am doing great. I love where I am living and I love the staff at ACI. I am so grateful for the fellowship I have with my peers.
